Please limit your nomination to 350 words. All parts of your nomination must be complete for judges to review.

Your description should be as specific as possible. Please include:

• The name of the organization for which the nominee volunteers and the number of hours volunteered each month.
• A description of the volunteer duties the nominee performs.
• An approximation of the number of people who are helped or affected by the nominee through their volunteer work and a description of how their efforts impact the community.
• A description of anything unique, outstanding or heroic about the nominee's actions and character (i.e. acts of vision, dedication, courage, tenacity, etc.).
• If available, a link to the web site of the organization for which the nominee volunteers.
